4.9 My Health Care

4.9.1 Premium Information

This option will display for BadgerCare Plus members age 19 to 64 who do not have dependent children under age 19 living in their home and who owe a premium. Members can click Premium Information to see how much they owe and to make a payment. See Chapter 10 Premiums for more information.

4.9.2 Request Explanation of Medical Benefits (EOMB)

A person may need a statement of the medical services that have been received through his or her health plan. This statement is called an "Explanation of Medical Benefits" or EOMB. To request an EOMB, click Request an Explanation of Medical Benefits (EOMB) under My Health Care on the Account Home menu. (An EOMB can also be requested through a tab at the top of the Health Care Details page. See Section 4.2.9 Get an Explanation of Medical Benefits for more information.)

The primary personThis is the person who applied for benefits. Sometimes this person is also referred to as the head of the household or case head. or the spouse of the primary person can request an EOMB for anyone receiving health care on his or her case. A person who is not the primary person or is not the spouse of the primary person will only be able to request an EOMB for himself or herself.
